Use this checklist to identify potential fall hazards and accessibility barriers in your home. If you check "No" on any item, consider contacting a certified contractor for a modification quote.

1. Exterior & Entryway

Is the walkway to the front door free of cracks and tripping hazards?
Is there good lighting along the path and at the front door?
Are there handrails on both sides of any exterior steps?
Is the doorway at least 32 inches wide for wheelchair access?
Is the threshold flush (no step up) or ramped?

2. Bathroom (High Risk Area)
